Chronic Care Management

Chronic Care Management by ChartSpan offers care coordination services to patients covered by Medicare. The program gives patients access to healthcare professionals seven days a week, 24 hours a day, and 365 days a year. 

There may be a co-pay associated with this program, and deductibles do apply.

Program Benefits

  • Access 24/7 to a team of local medical professionals.
  • Personalized care plan focused on achieving patient-guided long-term health goals. 
  • Help with medications, scheduling appointments, arranging transportation, and even setting up meal services. 
  • Sending messages to the providers for refills and helping find discounts for expensive medications. 
  • Answering questions patients may have about their healthcare. 
  • Requesting and storing records from other providers to give the patient's provider a better look at his or her overall health. 

The goal of the program is to keep patients healthier by staying in touch between appointments.  ChartSpan calls patients once a month. Patients may call anytime and patients are encouraged to call. Patients are not required to enroll in the program and may unenroll at any time.
